Survey For Valve Replacement Patients & Caregivers

Hey everybody,

I was curious to know if any of you would be interested in filling out a survey about your heart valve surgery experience. The survey is intended for both patients and caregivers.

The results of the survey will be published in the second printing of my book, The Patient's Guide To Heart Valve Surgery.

It's not a long survey. It was designed to be completed in 10 minutes. I am trying to better understand the patient / caregiver perspective on the realities of valve surgery in their lives - before and after surgery.

How anonymous would our replies be? Or what would identify/differentiate one reply from another if we want to remain anonymous?

My answer is, "Maybe," depending on the detail needed in the replies to the questions, I guess.

All information provided by you would be completely anonymous. I do not intend on referencing anyone in the analysis unless the respondent agrees to be identified.

It's actually the first question on the survey.
