I am 38 yrs old and was informed 3 weeks ago at the Cleveland Clinic that I shall need OHS in the upcoming future, undetermined time as of yet. I was there alone -- my wife was ill and my mother has cancer. The news was a huge shock to me. Prior to this, I had only had mild aortic regurgitation in a trileaflet aortic valve since 1990 (age 20)- last echo in late 2004 said same and I was only supposed to get echos every 3 to 5 years. Recent two echos, one at home and one at CC, state I have 2-3+ aortic regurgitation, a fused bicuspid aortic valve, mild regurgitation in all other valves and a slightly dilated left ventricle. Additionally, on Friday I was told by the infectious disease at CC that I have extremely high antibodies to chlamydia pneumoniae (spread thru common contact) and that this could be a culprit so I am starting a 6 month antibiotic therapy for this.

One of the decisions I have to make is about which cardiologist to work with at the Cleveland Clinic going forward. I need to communicate the latest finding to him. I live in Latin America and while in my country, healthcare is fine for day to day things, the things I have are beyond the scope of expertise of local cardiologists (my local cardiologist even mentioned not having much experience in valve related issues). Therefore, my local cardiologist and I may need to be in more close contact than the average patient in the US.

Upon receiving news from the first CC cardiologist (cardiologist #1) as to my diagnosis, I developed a list of questions for the second appointment. Cardiologist #1 told me he only had 3 minutes for my questions and otherwise to follow up with his assistant and that he'd get back to me. I felt I had little access to him and worried for 10 days (slept 3 hours per night and lost 10lbs in 10 days).

The following week I was advised to return to the CC for additional infectious disease testing. I had a follow-up appointment with cardiologist #1 but upon expressing my past experience to the International Patients people, they changed my appointment to a new cardiologist without confirming with me. The new cardiologist (#2) is a colleague of #1s and even shares the same clinic with cardiologist #1 and started the appointment by asking me why I was seeing him instead of cardiologist #1? he seemed a bit disapproving of me seeing him if I had seen #1 already and they are colleagues. Cardiologist #2? s advise to me seemed a bit more matter of fact that cardiologist #1.
In addition to my diagnosis, I am also now concerned about whom to proceed with in terms of cardiologist there, #1, #2 or a third one? I am also concerned that #1 and #2 may be upset and I certainly don?t want any personal things getting in the way.

Any thoughts, ideas or suggestions?
Has anyone had any experience or heard positive or negative information about any of the following cardiologists at CC: Allan Klein, Richard Grimm or Brian Griffin? I suspect all cardiologists there are anywhere from good to great. Thanks.

I had Brian Griffin as my cardiologist. I had a aortic valve replacement at the Cleveland Clinic 5+ weeks ago. Dr Griffin was very good with me in that he spent a good deal of time talking with me about the various options I had available. Ultimately he recommended a mechanical valve for me. It was my understanding that Dr Griffin is a valve specialist with 20+ years experience. I was pleased in that he was very personable and understanding to the fact that I had never had any type of surgical procedure before, ever ! I think we spoke about 45 minutes at my first appointment. He even had me laughing a few times and I was NOT in a laughing mood when I got there.

I don't know the other two doctors you had listed but I give Dr Griffin a big thumbs up !

we have many members who had surgery at Cleveland Clinic. We have the clinics all over the U.S.

Members will be coming along any moment with much information and opinions. I don't believe we have had any unsatisfactory statements from member - in my memory, that is.

Thanks for your inputs but believe me a 3 hour flight to the US is more than well worth it to receive the best care in terms of damaged heart valves. I travel internationally about 35 times per year (that's more frequently than every other week) on work so why wouldn't I go to the US for my heart? In my country, valve replacements do not occur...I imagine many people just pass away...so, in light of this, what is a 3 hour flight to Houston or two three hour flights to Cleveland? I do not believe that care for a potentially deadly ailment should be driven by convenience but by where the best care is. Perhaps if I was in Boston, I'd stay there and go to one of the fine hospitals there; but being in Latin America, you pretty much have to travel anyways.

The difficulty is that I may need to manage my cardiological care more closely with my US cardiologist because my local country cardiologists are less experienced with the matter. This is what has been difficult, what I think cardiologist # 1 above or #2 had a hard time understanding. Make sense? Any thoughts?
 
I think being in Latin America may also place me in a tougher spot if I ever have problems with a valve replacement or bleeding from Coumadin etc... but that is something I'll have to figure out later.
 
since you are traveling that much per year, you should probably not have a problelm getting yours done in the U.S. at all. Maybe continuing to work with these doctors is the best plan. Since they are both highly qualified to do the job, you should be safe with one of them. I wish you all the best. Keep us posted.

As mentioned, Houston has great heart health care. Patients come from round the world to Houston for heart care and surgeries. I have a nurse cousin who worked in the heart field and surgery floor of one of the Houston hospitals and had great things to say about it. Dr DeBakey practiced there and if he is still around, he's wandering the halls to oversee.

IF you receive a mechanical valve, you will of course be put on Coumadin. One of the Best places to learn about living with and on Coumadin is Al Lodwick's website, www.warfarinfo.com

Home Testing and Dosing has been found to be the BEST way of monitoring and managing INR so you will probably want to purchase your own (finger-test) instrument.

Rick any cardiologist at CCF will be fine unless you need specific things like language interpretation or such. I had Roger Mills, but like I said, they are all very good Cardiologists. I couldn't put my finger on just one. If language is a barrier for you or your home cardiologist, just talk to them at Cleveland about your concerns and they'll line you up with someone that can do it all for you.

That makes perfect sense.

When Joe first got his aortic valve replacement many years ago, there wasn't a cardiologist where we live that could help him, since valve surgery was rare at the time.

So, he traveled to Boston, Massachusetts to Lahey Clinic and saw a cardiologist there. That was a 3-1/2" hour car trip for us. It was well worth it. We went on that way for many, many years until valve surgery became more popular and cardiologists around here knew what they were doing.

Dr. Grimm is my cardio and I have had nothng but great luck with him. He answers every question and gives me all of the time I feel I need; even on the telephone.

Sorry you had a bad time at the Cleveland Clinic. I had my aortic surgery done there two years ago at age 38. While I truly believe I got the best care possible and my treatment while an inpatient was top notch, I admit that they can be somewhat lacking in the "soft skills" of making a patient feel known and personally cared for prior to the time when surgery is needed.

Because I wasn't pleased with the communication I had with my cardiologist pre-surgery (although, again, I think the medical care I received was fabulous), I switched to V. Kalahasti after surgery and have been very happy. He always has time to answer all my questions, has a terrific sense of humor and is very good at explaining things in a way that a lay person can understand. Based on what he has said, I think he prides himself on taking time to talk with his patients.

Well, cardiologist #1 apparently prefers I see a different cardiologist. I only met him at the actual consult and had no issues with him then. However, all future (mis)communication was 1 or 2 people away and mishandled this way (I never talked to him again). For instance, I would request to talk to him to explain myself and ask questions (couldve even been by phone) and instead I would received a response via email that he felt I did not need an additional consult for a while. He apparently called me directly at one point to my home mobile which I left in my home country before travelling though, and I did not find out until I returned. The MD was good, just the communication was mishandled by admin intermediaries and bad circumstances.

So this makes my decision simple.

I actually feel I received good care at Cleveland Clinic overall and think pretty much any MD working there must be at least good. I just feel that communicating via one or two other people created a mess in this instance.

since you are flying in from latin America why not go to the Cleveland Clinic Weston Florida? You can most likely fly into either Miami or Ft. Lauderdale and it is a short drive to CC.

Dr. William Stewart Was Unbelievably Fantastic. I Loved Him. He Was On Eof The Most Down To Earth Doctors I Have Ever Come In Contact With. Very Personal. He Took The Time To Call Me At Home To See How I Was Doing Prior To My Coming To Ccf For Surgery. I Highly Recommend Him. :d
