Deb, if your surgery is cnx, can your doctor try to contact my surgeon to perform the operation? I believe that your provincial health care should be willing, now, to pay any and all costs associated with your surgery no matter where you choose to have it. I really think it's time to check in with the province on this and get authorized to have your operation anywhere you can get in first and fastest.

I think, don't quote me, the Public Service Health Care Plan has clauses built into their contract that allows for travel (even out of country) in these situations, too. I'd be calling up the provider and inquiring as to pre-auth. Then book your vacation along with your surgery. :)

Take Heart dear lady, I'm sure someone will have the answers.

Hey there Deb, I think it was already mentioned that you should start your daily dosage, as it will take 3 to 4 days before it shows.
I think if you were to "up" your dose your INR will be to high in the days to come.
Someone will be along to tell me I'm wrong - but this is just my opinion and I thought I read it somewhere if not on this thread.

Hi again, Debbie! You take the Lovenox WITH the Warfarin as the Warfarin builds proper (therapeutic) levels in your body which takes 3-4 days thereabouts.

IF IT WERE ME - and it was just recently - I would be taking the prescribed dosage of Lovenox twice a day.

I am not sure why your doctor is leaving it up to us, even though we are all very much concerned and empathetic - he needs to be telling you what to do and stick with it! Not trying to be a b***h about it, but wow...

Lovenox and starting Warfarin work together.

I was at 1 yesterday trying to get Coumadin underway, too...sort of freaked as that's "thick," as she put it, and it's not "thick" just not therapeutic level.

Hope you can get a straight answer out of your doctor. This is unfamiliar territory for me as my cardiologist gets back to me ASAP on anything like this.

I know that's a rare thing, so I really appreciate that about him.

Many ppl don't like the idea of "loading" doses as they can cause a spike and dip effect.
I hope you are still taking your Lovenox needles, sometimes it takes a few days for the INR to catch up.

OLD Teachings were to take a "loading dose" of Coumadin (50% higher than normal) for 1 or 2 days, then resume your normal dose when coming up for NO anticoagulation. This technique can sometimes result in 'overshooting' your target range and putting you into an INR Rollercoaster ride. NOT GOOD.

NEW Teachings are to use Bridging (Lovenox or Heparin) and take your NORMAL Dose of Coumadin. Continue the Lovenox until your INR is back in range (usually several days). Test INR after 3 or 4 days. Note that Lovenox does NOT affect INR because it operates on a different part of the coagulation mechanism.

Once your INR is back in range, you can discontinue the Lovenox. Until then, the standard protocol is to inject your Lovenox shots into your 'belly fat' every 12 hours.

Hi Fran my doc, straight out says he's not great at dealing with this mess

doesn't wanna create a bleed and we don't have any better options of

doc's here although my cardiologist is gone till end of next week and

possible relocate for him and i,ve hardly had interaction with this cardio

only started in sept2008 and they just won't turn it over to someone

unfamiliar as same happened to my surgeon when he got hit by a car

in canada they have to reasign your case you could die while waiting

for them to reassign, and thats 5 or 6 hours away, from me which means

in my town theres no heart surgeons or cardios.Another town where freddie l

lives hour away, but been down that road and wasnot good for me or them

they,ve suggested i go where technology for me is more readily available

i'm kind of a difficult case for them and scare them with each sugery.:p

But i dont live a boring life with my luck and its been this way for me

all the time:p:Dnever a drag.

Thanks Al this is very good to know and basically answers my question

starting to enjoy the injections luv the thrill of belly piercing:p:D

Zipper if his intentions are to bring you back to full anticoagulated, then I would only restart my regular dosing. As Al mentioned, that loading dose thing is likely to to overshoot and then he'll want to slam the brakes on to bring it down. Nice and easy gets it's just as fast with no overshooting. You had 7mg yesterday, so normally, you'd do 6 today right? I would just continue my regular dosing and you should be able to stop the Lovenox Saturday or Sunday night. You should be at least 2.0 or above by then.
