This is an exact copy of an e-mail sent to me. The person is nurse who said that she got the job because they liked the fact that she had taken off only one sick day in alomst 20 years at her previous job.

"I was hired 6 months ago by an internist group to manage their anticoagulation clinic. Was given 1 protocol sheet & 1 algorithm sheet,250 patients & no training. I handle all dosing. Scarry isnt't it?"

She bought my books and algorithms at her own expense. Just think what it would be like if she wasn't interested in learning.
 
oh my goodness. That REALLY IS scary. At least she will try her own learning techniques (yours, actually) and her company should be very grateful to her.
 
At least she recognizes that she is not a good manager and she is trying to become one. How many don't know that they aren't and are not trying to get better?
 
Ah come on Al, you know they are Gods and know more then anyone else. They'll tell you so too, well some of them will anyhow. :D

There's also the God-by-Proxy people. They are the nurses and PA's that also think they know it all since the doctor put them in charge of the Coumadin patients.
 
That just proves my point. 95% of the nurses I have come in contact with for anticoagulation management did not know what they heck they were doing! They treat patients one in the same. By the "book". As we are aware...it's not one dose fits all. It is also not wise to tell eveyone to hold their dose for 2-3 days for an INR of 4.0! Yo yo, yo yo, Scary, yes indeed!
 
When I had my AVR, there were two primary Anti-Coagulation Clinics in Huntsville, Alabama.

One at The Heart Center (affiliated with Huntsville Hospital) and a stand-alone Coumadin Clinic, also affiliated with Huntsville Hospital. I chose the Coumadin Clinic with it's Pharm D Director and very capable CRNP head nurse.

Those two facilities have since been combined, there have been several changes in staff with 3 new CRNP's and several new nurses. I have been VERY IMPRESSED with these new Nurses and CRNP's. Obviously their training (based on the CHEST guidelines) has been extensive.

Their recommendations have been in line with what I have learned from www.warfarinfo.com and the sage advice of offered by Al Lodwick and the experienced and knowledgable members of VR.com

I feel I have been doubly blessed in my Anti-coagulation Management Education. Hopefully these good experiences will proliferate through education of more practicioneers in the field.
