Pre-employment health questionnaire

Does anyone know what types of questions that they ask on a pre-employment health questionnaire? I may be getting a job offer either this week or next and I have been told I have to do a drug test and a pre-employment health questionnaire. The drug test I'm not too concerned about (other than I have to list all my RX's), but the questionnaire has me a little concerned. Does anyone have any first hand knowledge of this?

Basically they can't ask you anything other then for the job your applying for such as, Can you lift x amount and carry. Can you sit walk stand climb or whatever the case maybe. Stuff like that.

Speaking of this, has anyone seen the movie Gattaca? I imagine they'll be testing genes eventually for insurance and employment decisions.

this question has come up in the past but I can't recall who needed info. the main question was 'shall I tell them about my valve replacement'. Maybe some of them will see this and drop by this thread.
