need advice

Need help with inr.Had valve replaced 2007,after surgery delay for
pulmonary embolism.Lately my swings in inr are from 4.8 to the 1.9
today.So i was sent to hospital for heparin injections last night and
today,and inr checked sunday.My warfarin was only adjusted by 3mg
a week when it was high.So my question is should i be overly
worried are just wait for the pros to adjust me back into range.

You should not rely on Heparin to get you back into your range. The Heparin will only have effect for one day after and injection. A better approach is to adjust your doseage,

they have adjusted my warfarin adding a dose 11am of 7.5 mg and told
me to go to 5mg day normal dose time 5pm 35mg per wk up from 30 weekly
plus the heprin shots going back to hosiptal sunday any respones will
help me

Most people around here recommend 10% adjustments. Yours was around 16%, but for the 1.9 INR that you mentioned in the other post, that's probably appropriate. Have you ever been in range for any length of time?

Have you made any major changes in your diet, exercise routine, supplements, other prescriptions, Warfarin brand, etc.? It is very unusual to be stable for two years and then have a sudden spike. That happened to me several months ago, and knowing that nothing else changed, I wrote it off to my changing hormones as I age. (I'm 47)

You were wise to cover yourself with Heparin shots until the INR is raised.
Can you post your daily dosages and INRs for the last week or so?

So for today they have told you to take 2 doses.....7.5 this morning and 5 mg this evening?
We usually like to spread out the increase over a couple of days....

Either the 4.8 or 1.9 may have been a "bad" test and gave you an incorrect reading, and hence, you are making adjustments that will cause a yo-yo effect (hi then lo then hi etc).

I have never experienced warfarin going way out of control (4.8 to 1.9) in a month without something besides warfarin causing it. I agree with Lisa in Katy, has there been some other change in your life, especially the introduction or discontinuence of medicine.

I would prefer that my doc make small warfarin dosage adjustments until I got back in theraputic range, especially if you have been stable and this just happened "out of the blue"
