    My surgeon said go for antibiotic cover because it's a lot better than endocarditis and if you've had the antibiotic before you should be fine. This is in the UK - I don't know whether the dentists can still prescribe the antibiotic or whether a visit to the Doctor would be required here.

    Yep, so long as the relevant bits aren't diseased or cancerous, and so long as you don't have HIV or hepatitis. With most of us I imagine they're not going to bother with the heart.
