My range of 2.5-3.5 has been stable at 73.5 mg warfarin per week.

What will an increase of 3.5 mg for one day cause?

Had I been self adjusting I would have upped it by .5 mg on one day only.

However my new XS read .8 above the veinous draw so I'm wondering could this be the reason for such a large increase.

Weather is snowy, blowy and cold. Brrrrr
 
Lance, I'm sorry, I don't quite understand your question.

Your lab INR was 2.6 and you're wondering whether to increase your dose?

And your XS INR was 3.4?

I'd just keep to my dose, if I were you. (If that's the question you were asking?:) )

XS reading this a.m. on my XS was 2.6. My range is 2.5-3.5 and I'm usually in that range at 10.5 mg warfarin a day.

My warfarin manager has advised me to take 14 mg. to-day followed by the usual 10.5 mg for the rest of the week because of the 2.6 reading.

When we tested my XS at the same time a vein draw was done my XS was .8 higher.

I'm surprised the increase is more than .5 and thought I'd check it out.

My INR has been decreasing over the past 3 weeks.

I take it you have been dropping with the 10.5mg/day dose? If so, it sounds like a slight increase might be in order. The amount of the increase suggested sounds okay and certainly should not increase you above your range.

I would definitely retest in one week.
 
Hi Lance,

If you are taking 73.5 mg a week then you are nt the normal atient as regards Coumadin metabolism. Regardless, 0.5 mg would probably not do anything at all. The dose of 3.5 mg increase suggested falls within the 5% increase usually recommended.
