Effective December 15, 2006, QAS will be contracted as an in-network provider with Blue Cross/Blue Shield of FL.
As a result, testing supplies deemed medically necessary by your insurance plan will no longer require full payment from you prior to shipment. This new status will enable you to order supplies with little or no cost, based on your co-payment or deductible. Please note that the final determination of benefits will be handled by your Blue Cross/Blue Shield insurance plan.
In most cases, the following will apply:
A prior authorization will be required prior to shipment (obtained by QAS)
QAS will continue to ship testing supplies to your door
QAS will file a claim for direct payment following your shipment
Copies of your explanation of benefits will be sent to QAS
