Joe has Heparin sensitivity. They are using Heparin during his dialysis treatments, and he is also an at risk candidate for more brain bleeding, which caused his stroke (I believe) while on Heparin bridging in the hospital, recently.

What do people in this scenario do?

Nancy, like so many other situations, Joe is a ground-breaker. Nobody knows what to do because it is hard to recruit enough people like him to do studies. In these situations, medicine becomes an art rather than a science. You have to do what seems right for each individual patient.

Reason I asked was because Joe was exhibiting symptoms of possible brain bleeding again, lethargy, hallucinations, agitation, weakness. I brought him to the ICU today.

His CAT scan showed a stable brain situation thank God.

The culprits are a very low bloodpressure, high ammonia levels (cardiac cirrhosis), and high CO levels, at least so far.

He has been admitted to the ICU again, but I believe it will be a relatively short stay. They have to get several things straightened out.

But the worst case scenario was leakage into the brain, and that wasn't the case.

I'll start another thread tomorrow when I have some time.

Hi Nancy -

I saw that you had posted so late in the evening for the east coast and so checked the thread. I hope things will be much better tomorrow for you and Joe.

Adding a P.S. edit here: Could the symptoms Joe is having now be related in any way to the purple toes from several days ago?

DearNancy:

I am so sorry to hear that Joe has had another set back.

FYI - When we were at the surgeon's on Tuesday, we talked about the need for bridging Wayne prior to the surgery. He said they no longer do heparin, as over the past few years they have seen many people have problems with the heparin, and blood issues. This is a recent problem. They therefor have switched to lovenox injections, ceasing the coumaden. Frnakly it makes me nervous.

Best wishes for you and Joe.

Marybeth

Nancy, you and Joe are often in my prayers as I know how very complicated your lives have become with his medical problems. I have no advice whatsoever other than pray, follow your heart, and take it day by day.
