I had an arthroscopy last year for a "bucket handle" tear in my cartilage. What is normally an overnight procedure for most folks, turned out to be seven or eight days
The length of time is to:
a) let the INR come down a bit
b) bridge with heparin for the procedure
c) get INR level back to normal

This is despite me self-testing. They wouldn't let me manage my own INR post-op.

Have you contacted TGH where you had your heart surgery?

They will know if "bridging" is required and are able tell you how it's done.
