I use a 5 X 8 inch spiral notebook to keep track of my Doctor visits.
Before each visit, I list any concerns / questions which I show the Doc. This saves time over verbally going over each question and allows the Dr. to see to whole list at once, quickly answering the short issues and taking more time with the more significant issues.
