Dear All,

Seen many drs in Bangkok in regards to cardiology; my main one is highly regarded, does TAVI, trained at Augusta and Emory etc

Just did a 2nd opinion visit with a surgeon

A lot of this is in shorthand; let me know if you have questions

I sent a few of his suggestions off to my main surgeon/heart dr for guidance

Recommended:
Recommends Inspiria Resilia-- can last 19-21 years coated to slow down calcification
TAVI can last 15 but higher risk of stroke
Do chromosonol test to check for damaged chromosomes
ETA for AVR: 3-5 years
Can go back to USA when severe and do AVS then come back to Asia
yearly treadmill
Yearly echo

Surgeon:
Anyone can do a mechanical valve; tissue TAVI harder to do more costly

If Persistent chest pain
A) Treadmill yearly
B) CT Scan of Coronary Artery to check for calcium score + anatomical study of coronary artery

Future:
Moving away from needing blood thinners
Future of AVR: TAVI then tissue valve/can correct other issues at that time

May want to do aortic study

Did additional training at Leipzig, Houston, Cedar Sinai Beverly Hills

Criteria for surgery/cannot fly back to USA
1)Severe
2) In HF
3) Syncope
4) Severe chronic pain

Feedback:
Valve opening for normal person 2-4; mine 1.35; when sub 1.0 AVR needed
Velocity 3.47
AV Max 48
Max PG 48
Mean 28 Valve Gradient
Pressure across the valve key indicator
I am mild moderate IHHO
LVH minor no big deal
My current doctor is good
Bangkok Hospital, Phaya Thai and Bumrungrad best private hospitals for surgery
Sriroj/government hospitals good but busy and post op may not see original surgeon; with
private hospital will see dr from start to finish but cost higher
Gov't surgeons do tons of AVR
Hard to know complications until they cut you open, run pre op tests, and things may change as you age.

AVR:
After 3 weeks sternum pain gone
Will need nurse and/or friends post op up to one month
12 weeks and sternum is 100% fused healed
Can take NSAIDS for pain even on blood thinners but NSAIDs bad for heart, kidneys, and esophagus; goal is
to move towards natural treatment of pain: heat, cold, acupuncture, massage, lasers,

What causes contributes AVS:
Genetic abnormality
Calcification-duh !
Stress-inflammation--damage to heart
Bacterium from dental gum disease
High TC/LDL
Diabetes
Inflammation
Chromosomes f__cked up
Does not agree that lowering lp(a) slows AVS or CVD"
