Inflammation of periodontal tissue leads to deepening of the gingival crevice and formation of periodontal pockets, acting as a reservoir for a large number of microorganisms. Periodontal infections affect the oral tissues, in addition bacteria may get entry into the bloodstream via ulcerated inflamed crevice and pocket epithelium, and the adjacent gingival microcirculation. Invasive dental procedures and normal daily activities, such as chewing and tooth brushing are the important predisposing factors for entry of microbes into bloodstream (7, 18-22). Bacteremia and low-grade inflammation because of periodontal infections may carry a risk for systemic conditions. It has been observed that periodontitis is one of the causes or consequences of developing atherosclerosis. Microorganisms or their products present in circulation may promote pathogenesis and enhance local inflammatory changes in vessel walls and thereby may promote process of clotting and clot formation (23). The periodontal disease has also been reported to be an important risk factor for nonhemorrhagic stroke (24). Several other studies have also found that there is the oral-systemic link in various conditions like infective endocarditis and cardiovascular diseases, premature low birth weight deliveries, and diabetes mellitus (25-27).

So sorry you’re going thru this. My 3rd surgery was the result of a nasty bacteria, staph. lugdunensis. I also had pockets and Dr Miller at Stanford said the tissue around my mechanical valve was in bad shape. That’s where it was leaking after 2 stays in the hospital and 6 weeks of picc line and oral antibiotics. My bacteria was gone but the damage was done. A week after my flu like symptoms I had already had a brain bleed from inr of 9, put in ER for DIC, but my blood cultures results showed the bacteria so I was treated. When it came to surgery, my doc said he’d prefer to replace my mech aortic valve with another mech. However, he really wouldn’t know how bad the tissue was until he got in there. So he also prepared me for a possible homograft which, in 09, lasted about 10 yrs average. I was thankful that my 3rd surgery went well. The aortic valve/graft they had created for me during surgery was placed higher up the aorta and all went well. I also needed mitral repair from the bacteria issues so that was done, too. I was 54 and awoke feeling good! The infectious disease doc didn’t know how I got that bacteria.

Thank you for sharing your experience! I am 54 and had my first surgery November 29, 2021 to replace a Bicuspid Valve, aneurysm of 4.5mm and two Coronary Artery moved.

I'm so glad you had a good outcome! Did you end up getting the mechanical valve? I hope to get another mechanical if possible, but my surgeon said, due to the tissue damage he might have to place a Homograft. I'm going to see if I can deal with the damage from my current valve. My partner and I hiked yesterday and my breathing wasn't great. The IV antibiotic has so many side effects and I believe I am experiencing many of them. The 6 weeks is over Tuesday so I'm hoping to get the PICC Line removed Tuesday and then put on oral antibiotics. I will be able to tell if I can live with damage or not. So glad to hear your story! 🙏🏻

I'm so glad you had a good outcome! Did you end up getting the mechanical valve? I hope to get another mechanical if possible, but my surgeon said, due to the tissue damage he might have to place a Homograft. I'm going to see if I can deal with the damage from my current valve. My partner and I hiked yesterday and my breathing wasn't great. The IV antibiotic has so many side effects and I believe I am experiencing many of them. Yes, I got another mechanical valve in graft since I had a graft from ‘89 in there. They basically replaced everything that had been done from my previous surgeries. My only lingering problems are chronic a fib and mod to severe mitral leakage. The mitral repair often only lasts approximately 10 yrs so that’s a bummer. My new cardiologist has upped my meds so I won’t need another surgery for awhile he hopes.

I'm so glad you're doing well and pray you won't need another surgery. I can't believe the bum luck on this entire situation. If I had it to do over I would have waited as long as I could before doing the surgery this past November. I never got back to who I was prior to surgery and then a blood infection to destroy the entire valve. Hindsight is 20/20
