Since surgery my meds have changed a bit and am on furosemide40mx2
Amiodarone 200mg x once aday.metropololhalf tab x2 daily.


warfarin needs to be in range daily between 2and 3.2
this is what ive taken in last days feb.5 was 3.2 took 6mg of warfarin
on feb 6th brought me to 4.0inr and took 2mg yesterday of warfarin.
Today inr at 4.5 and i'm wondering if i should take any or skip for tonight?

Ive been told the Amiodarone at 200mg once a day can play some haveic on the warfarin to due to changes of dose which changed upon my return home from 4omg to 200mg

Someone could please help suggest todays inr 4.5 kinda high for me and maybe yesterday shoulda skipped but took 2 mg.Thanks everyone again.

Only problem that if you skip completely today's dose, you may drop too low.

To me, 4.5 is not that high; what I would do is 1/2 today's dose & then continue on your regular dose & check in 4 to 5 days & see where you're at.
 
Zip, don't skip any doses....you will start your INR swinging. 4.5 is nothing to freak about.
Recap:

Feb.5 ---INR 3.2----6mg
Feb.6---INR 4.0----2mg
Feb.7---INR 4.5----
What you are seeing today is the dose from 2 or 3 days ago (6mg).
Yes, take some today. Take 2 or 3 mg like Norma suggested, and test again soon.

You're a double-valver and it's not unheard of for those people to have a 3-4 range.

I'm not quite sure about the 3.2. Those numbers give me a chuckle because it makes people think that the INR is an exact number and there can be at least .3 variation on any given test. So your INR could have been 4.8, or 4.2.

Point well put Karlynn because my cardio is perfectly happy if I can keep it in the low 4's & he's not unhappy if it's at 4.5, because of the fact that I have double mechanical valves!

Going from 40 to 200mg might cause a warfarin adjustment. However, I kinda think you may be testing too quick for an adjustment to take effect. The 2mg on friday 2/6 would not show up in your 4.5 INR on saturday 2/7. Like Norma says, reduce your current dose only a little and retest monday or tuesday. Continue small dosage adjustments until you get back in your rang. An INR above 4 is only a minor problem. Big changes, based on quick testing, will cause the "yo-yo".
 
Your going to have to find your new sweet spot now that the Amiodarone is increased. Your INR will increase as well. Don't test everyday! No more then twice a week, preferably only once. You've got to give the warfarin time to give a true picture of where your really at.

My family dr. chuckled too Karlynn at the 2 to 3 range for double valve

He didn't think that made sence at all,but in hospital they freaked out if

i went higher than 4:rolleyes:

Thanks all and Bina i feel like the train still ran me over,soooo weak,so

tired and eating,resting and only on tylenol extra strenght for pain,

sometimes doesn't seem enough,but im coping,food is crap and i walk,rest

eat what i can,rest some more,i think this ones gonna take me awhile

guys to get stronger.My mom is still here bless her at 83 a shame when

they have to look after us in this shape.

Well k i will take 2 or 3 mg warfarin tonight and not skip,check inr next

Ross how much time to find my favorite # and find now the low side of
warfarin is working. I used to take 7 and 8 mg daily before surgery to = like
54 mg weekly now im dropping to 6 and maybe a 7 to = 43 mg week
The amiodarone is sure a nausea hitter and later on hoping they can switch me to something else(YUK) what a terrrible drug i find it to be with or without food.my inr is 2.6 today but still working on doses prior to 3 days ago which were lower at 3 mgs,so guess it's gonna take some time to find my spot yet.
